(P-LE-1) Bloodbound Brilliance: The Symphony of Leadership Development in Rescuing Patient Blood Management from Peril to Triumph

Through cultivated leadership, healthcare organizations ascend from stagnation to innovation, pioneering a future where quality care thrives.Patient Blood Management (PBM) programs aim to optimize patient outcomes by minimizing unnecessary blood transfusions and promoting alternatives. Despite initial implementation, PBM initiatives in healthcare settings often face challenges in sustained effectiveness. This study aims to investigate the significance of LDPs in facilitating the successful adoption and integration of PBM practices, ultimately enhancing patient care, ensuring resource efficiency, and promoting a culture of continuous improvement within healthcare institutions

Study

Design/Methods: In a tertiary care teaching institution, a PBM program faced resistance post-implementation. An intervention involving a LDP targeted resident doctors, clinical specialties, hospital authorities, nursing, and technical staff to rejuvenate the PBM initiative. The study employed mixed-methods, including qualitative interviews and quantitative clinical data analysis. Success indicators of the LDP included robust interviews, focused group discussions, and Objective Structured Practical Examination (OSPE) and Objective Structured Clinical Examination (OSCE) assessments. Clinical data on blood transfusions, patient outcomes, and PBM guideline adherence were evaluated before and after LDP implementation

Results/Findings: Before the LDP implementation, the PBM program showed limited success with inconsistent adherence. After LDP, resident doctors demonstrated significant improvements in PBM values understanding (68.34%), communication skills (42.1% improvement), and clinical performance (bedside: 18.6%, academic: 11.5%). Nursing teams improved patient understanding by 27.3%, and technical staff enhanced hands-on skills (15.8%) and efficiency (TAT decreased by 4 minutes). Post-LDP, there was a notable reduction in blood component utilization: RBC transfusions decreased by 22.4%, FFP by 2.6%, and platelets by 24.5%. Pre-transfusion haemoglobin thresholds (9.1 g/dl to 7.9 g/dl) and platelet thresholds (27000/µL to 14000/µL) also decreased, reflecting improved PBM guideline adherence

Conclusions:

This study underscores the importance of leadership development in driving the success of PBM initiatives within healthcare institutions. By empowering resident doctors and clinicians with leadership skills and fostering a culture of continuous improvement, hospitals can overcome barriers to PBM implementation and achieve sustainable improvements in patient care.
